realism mutation?

Is it possible to play a mutation with realism on? i can't figure out how to do it. Thanks.

No. Only certain mutations have realism attached, so if you wanted to play for example chainsaw massacre on realism, you wouldn't be able to because the word realism is not attached to the mutation.
If it does have realism on the name of the mutation, etc, then it is realism only.
