Personally, mine worked fine for me. I have heard a lot of people having problems with these and we had done research on it before.
From what we found, there are two things you can try that have worked for some people...
1. Recover your gamertag on another xbox and then recover it on yours again.
2. Do the achievements on a Local Host. L4D1 doesn't give the option but there's a way to force the game to use Local Host. We used it when the servers were slow after the L4D2 release... Read the bottom of this page to learn how to do it. It's a good trick to learn for when the servers are busy...
L4D1 Force Local Server
Hope this helps!

Re: Crash Course Achievements?
I managed to get mine via making it a private game of versus. Worked fine. It apparently DOES NOT work online however. An oddity. The versus win does work online, but Jumping Jack Smash and Slippery Pull apparently are very glitchy.

Ah, I take it you do not have two controllers then? Perhaps you could borrow one from a friend just for the purpose of this.
A good place to get Jumping Jack Smash is actually RIGHT at the beginning of Crash Course. Keep respawning as an Infected until you get Hunter. Climb up the building DIRECTLY behind the crashed helicoptor and reach the top of the roof. This may take a few attempts, but providing you land on a survivor it should be an easy 25 Point Pounce.
As for Slippery Pull, this one is known to be glitchy, but without a system link game, it's downright impossible as far as I'm aware. Your best bet is to attempt again in a Versus online match. Just remember you need to pull the Boomer Bile covered survivor all the way to you, so Smokering from a roof will not work.
Getting the car pile up achievement is also a chore as the Tank only seems to spawn in the finale. Abuse that car in the corner, and make sure to hit the 3 CPU survivors with a car, hide as Tank, and then use the human controlled survivor to pull up the other three. Rinse and repeat until either a.) the survivors are all dead or b.) Tank is killed.

I wish you good luck. I'd help myself, but it'd be counter productive if it doesn't help them unlock.
